Alaska Island Community Services directly provides these services:
Care Coordination:
A trained social worker or nurse helps clients to obtain all available services
to help clients remain in their homes. The care coordinator maintains frequent
contact on an ongoing basis to ensure that, as needs change, services can be
'wrapped around' to provide support.
Respite Care:
Family members often provide a great deal of assistance to their loved ones who
may be CHOICE clients. Respite care can relieve a large amount of pressure on
family members by allowing them to take the breaks -- a few hours a day, a few
weeks a year -- that prevent burnout and preserve a loving family relationship.
Chore Services:
A chore worker helps clients in their home by carrying out tasks that may be be
difficult for them, from cleaning to food preparation.
